Supporting Local Dwyer Programs, Not Replacing Them

County Dwyer programs remain the foundation of local peer support. DwyerCare strengthens the statewide network by helping people reach the appropriate county program, agency, or community resource and by following through after the referral.

Make a DwyerCare Referral

You may submit a referral for yourself, a family member, or someone you are assisting professionally.

  • Complete the online form

  • Share the county and general reason for referral

  • Provide the preferred method of contact

  • A DwyerCare team member will review the request
